Titles will appear in your inventory and can be equipped by right clicking the item or moving the Title Item to the "RegularTitle" slot in the Title Tab of your inventory.

Oh, sorry. The npc Knight Captain Christiana gives them. You can't actually see them unless you're the proper specialty level. You CAN see them all if you're under level 10 though (at least as of last patch, they may have fixed it). Sorry for the late reply x.x

Actually, to get the Genius (God's Gift) and Legendary Genius (Legendary God's Gift) titles, you have to beat History Lir while under level 25, and Legendary Lir while under level 40, respectively.
