As for the Practice Matches, this is due to Season 1. Season 1 is a MESS, in terms of server stability. PVP.net chat is messing up. Lots of bugs with the PVP.net client itself. And a billion people want to Practice Match random builds and things, specifically with Xin Zhao. This puts more stress on the server. If you have only one player in a room, and hundreds of rooms, that's too much ****ing server space being taken up. So they want as many players in one room as possible. Hence, the 3v3/5v5 ranked/normal games.

Think of it like normal lifesteal. If you have a Vampiric Scepter, you have 12% lifesteal. With 100 damage per hit, you get 12 life back.
Spell vamp is the same way. With 10% spell vamp, you get 10% HP back of the damage you deal in magic damage, using your skills. (Some skills are physical damage, though). So, you would get back 100 health for a spell that did 1,000 damage. (You can obviously get more spell vamp)
But, if you have an AoE skill, like Anivia's Glacial Storm, your Spell Vamp is halved. So, if your Storm does 150 damage per second, you get 5% Spell Vamp. Which is 7.5 health back every time it hit for its full damage on anything. But it's AoE, so it would ultimate give more HP back than single-target.
