Refinement

What level range causes the item to lose 3 levels if failed and what level range causes you to lose the item if failed?

It depends on the color of the item.

I've only upgraded pinks and they start to break from +10 going to +11 and higher. I haven't upgraded anything since revolution so I can't help you confirm where the -3 starts to happen.

I've had the -3 levels a couple of times while trying to go from +6 to +7, if that helps

Rares/Uniques
0-2 = 100% Success
3-6 = -1 Level if Fail
7-9 = -3 Levels
10+ = Broken Item

Commons
0-2 = 100%
3-6 = -1 Level
7+ = Broken Item
