I haven't done "surveys" on Playspan or Ntreev, but I have done them before. And the ones that are working don't require you to fill out an offer before you can complete it. And you shouldn't need to give the "surveys" your credit card number at any point during the process. So basically, you can't seem to move on without completing an offer? It probably is no good and you should try a different one.
