Parts A-I are your personal particulars.
A - Name
B - Name
C - In-Game Name
D - Identification Number
E - Account Name
F - Email Adress
G - In-Game Name
I - Address
Parts 1-3 are only applicable if you are below 21 years old.
1 - Guardian's Name
2 - Guardian's Identification Number
3 - Contact Number
Parts 4-6 are for any account related issues. [ Will further edit them when i have the time to ]
Parts 7 & 8 are for attaching a copy of your Identification Card.
